Position Description

As an RN Case Manager, you will provide health and safety guidance to support and assure client and caregiver safety in the home environment. The RN will report directly to the Lead RN Case Manager.

  • Works as a TEAM with other RN Case Manager(s), Staffing Managers and Field Specialists to develop staffing strategies, resolve client issues, and develop/implement training procedures to elevate quality of care.
  • Perform Health and Home Assessments of Potential Clients.
  • Full Time or Part Time, Monday through Friday.

Qualifications

  • Current unencumbered Missouri State Professional Nurse License
  • Must maintain a valid driver's license and good driving record
  • The ability to function independently while making sound professional clinical judgment
  • The ability to assess and document client needs, and formulate individualized client care plans to meet those needs
  • Proficiency in Microsoft Office
